Online Forms
The Office offers secure online forms for individuals looking to:
make a protected disclosure of wrongdoing they believe is happening within the federal public sector;
make a complaint of reprisal; and/or
submit a request for funding for legal advice related to a disclosure of wrongdoing or complaint of reprisal.
To use these forms, you must have access to a valid e-mail address. Once you submit your completed form, it will be encrypted and sent directly to the Office.
For security reasons, after a period of more than 30 minutes of inactivity, your session will be timed out. You will lose all information and your form will not be saved.

This form is for both federal public sector employees and members of the public who wish to make a disclosure of wrongdoing that is believed to have occurred in or relating to a federal public sector organization. If your disclosure alleges wrongdoing by an elected official, please contact the Office of the Conflict of Interest and Ethics Commissioner who has jurisdiction under the Conflict of Interest Act.
Any member of the public may blow the whistle regarding allegations of wrongdoing that may have occurred in a federal public sector organization. Note that the Public Servants Disclosure Protection Act does not apply to hospitals, schools, municipalities, any organization falling under provincial legislation or the private sector. The Office therefore does not have jurisdiction over these sectors.
Public sector employees include: members of the RCMP and individuals working for a federal government department, agency, Crown corporation, etc. that is listed in the Financial Administration Act (Schedules I, I.1 to V). Excluded organizations are: the Canadian Forces, the Canadian Security Intelligence Service (CSIS), and the Communications Security Establishment (CSE).
Current or former public servants may file a reprisal complaint if measures were taken against them because they made a protected disclosure of wrongdoing or participated in a related investigation.
You may wish to seek legal advice before filing a disclosure or wrongdoing / making a complaint of reprisal or during the process, but you are not required to bring legal counsel, a union representative or any other type of representation. The Commissioner has the authority to approve access to legal advice to any person involved in a complaint or investigation up to $1,500 ($3,000 in exceptional circumstances).
